Job summary

IMPaCT Care is hiring Community Health Workers to serve PG County in partnership with Kaiser Permanente. This field-based role focuses on connecting high‑risk patients with care teams, addressing social determinants of health, and documenting interactions in the EPIC EMR system.

You are a trusted community member with experience in outreach or health education, comfortable navigating systems, and able to work Monday–Friday, 9am–5pm in the field across assigned regions.

Qualifications

High school diploma or equivalent required.
1–3 years in community-based outreach, health education, or related work preferred.
Valid driver’s license, must own a vehicle, have car insurance.
Ability to pass background check and pre-employment screening.
Schedule: Monday through Friday, 9am–5pm; field-based work.

Responsibilities

Build trust through regular, culturally aligned communication.
Identify and address barriers to care (food, transportation, housing).
Connect patients to care teams and resources; follow up as needed.
Document interactions and outcomes in EPIC EMR system.
Participate in integrated care huddles and collaborate with clinical teams.
