Job summary

IMPaCT Care is hiring a Community Health Worker to serve high‑risk patients in the Atlanta area, embedded within the Kaiser Permanente partnership. You will build trust in the community, navigate care systems, and coordinate with clinical teams to meet patient needs.

Qualifications

• High school diploma or equivalent is required.
• 1–3 years in community-based outreach, health education, or related work.
• Valid driver’s license, must own a vehicle, have car insurance.
• Ability to pass background check and pre-employment screening.
• Flexibility to cover multiple locations across the Atlanta area.
• Preferred: experience with EPIC EMR, Medicaid/DSNP knowledge, SDOH screening tools, risk stratification concepts.

Responsibilities

• Spend majority of time in the community visiting clients’ homes, hospitals, clinics, and community organizations.
• Coordinate with care teams and document interactions.
• Complete SDOH screenings and update care plans.
• Document client interactions and outcomes using EPIC EMR.
• Attend team huddles and participate in integrated care activities.
• Travel across the Atlanta region to meet client needs.
