IMPaCT Care is hiring Community Health Workers to serve PG County in partnership with Kaiser Permanente. This field-based role focuses on connecting high‑risk patients with care teams, addressing social determinants of health, and documenting interactions in the EPIC EMR system.
You are a trusted community member with experience in outreach or health education, comfortable navigating systems, and able to work Monday–Friday, 9am–5pm in the field across assigned regions.
At IMPaCT Care, we get out of bed every day to remake the American health workforce. We find, train, and enable Community Health Workers — caring people who keep their communities healthy. We've built the largest, most scientifically-proven Community Health Worker platform in the country, now active across 22 states and trusted by over 70 organizations, including Kaiser Permanente.
We are currently hiring Community Health Workers to serve PG County in partnership with Kaiser Permanente. If hired, you will work directly for IMPaCT Care.
As a Community Health Worker, you’ll be a vital link between high-risk patients and the care and services they need. You’ll do this through trust, cultural understanding, and hands‑on problem‑solving — working in the community, not behind a desk.
You are someone your community already knows and trusts — a neighbor, a peer, an advocate who has walked the path your clients are on. That lived experience is the foundation of this work. It's what opens doors that credentials alone cannot.
But trust gets you in the door — skills are what make the difference once you're there. You also bring real working knowledge of community health, social services, or peer support. You know how to navigate systems, coordinate care, and follow through in ways that actually move the needle for clients.
The combination of both is what makes an exceptional CHW.
This is a field-based position. You will spend the majority of your time in the community — in clients’ homes, in hospitals, in clinics, and at community organizations. You will report regularly to Kaiser Permanente locations for team meetings, training, and care huddles.
Salary: $67,252.50 per year.
