IMPaCT Care in Los Angeles, CA is seeking a Community Health Worker to serve embedded within the Kaiser Permanente partnership. You will work in the community to connect high-risk patients with care and social resources, building trust through culturally informed outreach and hands-on problem solving.

This full-time CHW role emphasizes coordination, documentation in EPIC EMR, and ongoing follow-up with clients to improve health outcomes.

Qualifications

• High school diploma or equivalent required.
• 1–3 years in community-based outreach, health education, or related work.
• Valid driver’s license, must own a vehicle, have car insurance.
• Ability to pass background check and pre-employment screening.
• Flexibility to cover multiple locations across the Atlanta area.

Responsabilités

• Build trust through consistent presence and culturally aligned, empathetic communication.
• Identify and address barriers to care: food insecurity, transportation, housing, and more.
• Connect patients to care teams and community resources; follow up to ensure needs are met.
• Document interactions, insights, and emerging patient needs using digital tools.
• Use feedback and performance data to refine your approach based on metrics.
