Job summary

Centene Corporation is seeking a Community Health Liaison to connect members with healthcare services, educate on preventive care, and help navigate benefits. This role involves home visits and community-based assessments to identify social determinants of health and close care gaps.

You will collaborate with providers, document interactions, and support quality initiatives while ensuring compliance with state and federal regulations.

Qualifications

• Bachelor’s degree or equivalent experience in health/social field.
• 2+ years in community health or healthcare quality.
• Experience with health plan members and navigating community resources.
• Familiarity with Medicaid/Medicare programs and quality measures (e.g., HEDIS).
• Strong communication and interpersonal skills.

Responsibilities

• Conduct outreach to identify care gaps and connect members with services.
• Perform home visits or community assessments to evaluate needs.
• Advocate for members; assist with appointments, care plans, and benefits access.
• Collaborate with providers to share quality data and support improvements.
• Educate members on preventive care and chronic condition management.
• Document interactions, care gap closures, and referrals.
• Coordinate with internal departments to improve outcomes.
• Monitor outreach effectiveness and care gap closure metrics.
• Maintain compliance with regulations and policies.
• Participate in campaigns and quality initiatives; engage community partners.
