Job summary

Centene Corporation is seeking a Community Health Outreach Specialist to improve health outcomes by identifying care gaps and connecting members with appropriate services. You will conduct home visits, perform community assessments, and coordinate with providers and internal teams to ensure timely, coordinated care.

Travel up to 50% and reside near Cook County, IL is required. The role emphasizes educating members on preventive care, navigating benefits, and documenting interactions.

Qualifications

• Bachelor’s degree in Social Work, Public Health, Nursing, or related field
• 2+ years in community health or healthcare quality required
• Experience working with health plan members and navigating community resources required
• Familiarity with Medicaid / Medicare programs and quality measures (e.g., HEDIS) required
• Strong communication and interpersonal skills

Responsibilities

• Conduct outreach to identify care gaps and connect members with healthcare services
• Perform home visits or community assessments to evaluate needs and social determinants of health
• Assist members in navigating care plans, scheduling appointments, and accessing benefits
• Collaborate with providers to share quality data and support improvement initiatives
• Educate members on preventive care and chronic condition management
• Document interactions, care gap closures, and referrals in systems
• Partner with internal departments to align efforts and improve outcomes
• Monitor outreach effectiveness and care gap closure metrics
• Maintain compliance with regulations and policies
• Participate in seasonal campaigns and quality initiatives
• Build relationships with local organizations and stakeholders
• Perform other duties as assigned
