Responsibilities include outreach, resource documentation, and supporting nurses and social workers with non-clinical assessments to refer members to appropriate services

Qualifications

• Requires a High School diploma or GED.
• Requires 1–2 years of related experience.

Responsibilities

• Connect members to known community and care resources in a cost‑effective manner.
• Coordinate community outreach resources and promote awareness of care/services.
• Support members on community and care resource inquiries and opportunities.
