Centene Corporation is seeking a field-oriented Community Health Worker to identify and close care gaps for members in the Illinois region. You will perform home visits and community assessments, connect members to healthcare services, and support navigation of benefits and entitlements to maintain continuity of care.

The role emphasizes collaboration with providers, education on preventive care, and documentation of interactions to drive improvements in HEDIS metrics.

Competenze

• Bachelor's degree in social work, public health, nursing, or related field.
• 2+ years in community health or healthcare quality.
• Experience working with health plan members and navigating community resources.
• Familiarity with Medicaid/Medicare programs and quality measures (HEDIS).
• Strong communication and interpersonal skills.
