Job summary

AbsoluteCare Medical Center & Pharmacy is seeking a Community Health Worker to connect members with social resources and enroll them in internal and external services.

The role involves home visits, non-clinical assessments, and collaboration with care teams. Requires CHW certification, 2+ years in outreach, and a valid driver’s license. Location: Pittsburgh area with local travel and schedule Mon–Fri, 8:00 AM–5:00 PM.

Qualifications

• High school diploma or equivalent.
• Must have a valid driver’s license, insurance, and reliable vehicle.
• CHW certification required prior to employment.
• 2+ years assisting vulnerable populations.
• CPR/BLS certification required.

Responsibilities

• Represent AbsoluteCare and explain services and resources to members.
• Conduct in-person non-clinical assessments and evaluate home environments.
• Create individualized care plans with clinical input.
• Connect members to community resources like housing, food, and transportation.
• Build trust and engagement with members and support systems.
• Manage a caseload, enroll members in services, and ensure follow-through.
• Document outreach in care platforms and EHR/CRM systems.
• Meet KPIs for member engagement, social needs, and enrollment.
• Support Member Lounge events.
