Job summary

The Community Health Worker at AbsoluteCare Medical Center & Pharmacy connects members with resources to address social needs and barriers to care, meeting them where they are—home, community, hospital, or clinic. You’ll guide enrollment, educate members, and support care management with a collaborative team in a matrixed environment.

Compensation ranges from $40,000 to $50,000 annually based on experience, with the potential for increases.

Qualifications

• High school diploma or equivalent required.
• Must have a valid driver’s license, proof of insurance, and reliable personal vehicle.
• 2+ years of experience with community agencies providing health care resources, advocacy, and education.
• CPR/BLS certification required.
• Certified as a Community Health Worker required prior to employment.
• Experience with vulnerable, hard-to-engage populations.
• Independent and persistent self-starter.
• Proficient in PC skills, such as Microsoft Office.
• Ability to travel locally at least 80% of the time.
• Knowledge and trusting relationships within the local community.

Responsibilities

• Represent AbsoluteCare and communicate how ABC’s services, programs, and resources can help in the member’s situation.
• Conduct in-person non-clinical assessments to understand member needs and barriers to care.
• Create and administer individualized care plans for members with clinical team input.
• Connect members with resources such as mental health, housing, transportation, and basic needs.
• Authentically build trust and rapport with members and communities.
• Manage a caseload, enroll members in services, and ensure follow-through on care plans.
• Partner with care team and attend daily huddles and meetings.
• Document daily outreach in care management platforms, EHRs, and CRM systems.
• Meet predefined metrics and KPIs for engagement and enrollment.
• Support members within the Member Lounge with events.
