Job summary

True Mental Health Services is hiring a Community Case Manager to help clients access mental health, medical, financial, educational, and social supports in home and community settings.

The role offers a meaningful field-based position with flexible scheduling, mileage reimbursement, and opportunities for bonuses. A Bachelor's degree in a behavioral health field is required, with Texas driver’s license and background checks mandatory.

Qualifications

• Bachelor’s degree in psychology, social work, medicine, nursing, or related behavioral health field.
• Valid Texas driver’s license and reliable transportation.
• Ability to pass criminal background screenings.
• Experience in community behavioral health preferred.

Responsibilities

• Conduct needs assessments and client evaluations.
• Develop, implement, and review service plans.
• Connect clients with community resources and services.
• Coordinate care with providers, families, schools, and agencies.
• Advocate for clients to obtain services and benefits.
• Monitor progress and adjust services as needed.
• Assist with discharge planning and aftercare coordination.
• Maintain clinical documentation within required timeframes.
• Participate in daily team meetings and case reviews.
• Ensure Medicaid compliance standards are met.
