Job summary

Dallasact seeks a dedicated case manager to deliver services under general supervision for county programs. You will identify needs, assess eligibility, and provide counseling, outreach and referrals to clients.

Responsibilities include interviewing clients, verifying documentation, preparing case plans and coordinating with other departments and community groups to ensure effective service delivery. Travel may be required.

Qualifications

• Bachelor’s degree required in Behavioral Science or related field.
• Prefers intake, assessment, case planning and evaluation experience.
• Must possess a valid Texas driver’s license with a good driving record.
• Skilled in use of standard software applications.
• Ability to communicate effectively, verbally and in writing, and to work with diverse stakeholders.

Responsibilities

• Interviews clients, obtains documentation and verifies information to assess needs and eligibility for programs or services.
• Analyzes client documentation to ensure eligibility and compliance with program requirements and laws.
• Provides counseling, referral and follow-up services to clients.
• Prepares case plans, summaries, briefs, requests, reports and other required documentation.
• Assists clients by coordinating with other departments, agencies, organizations and community groups to deliver services.
• Maintains knowledge of applicable policies, procedures and laws governing programs.
• Attends departmental meetings and trainings to broaden knowledge for better service.
• Performs other duties as assigned.
