Job summary

The Elevance Health Companies, Inc. is seeking a CFSP Managed Care Coordinator in Graham County. This field-based role requires frequent patient, member, and provider interactions across services, with hybrid work requirements and occasional office attendance.

Anticipated end date is 2026-11-20. You will coordinate integrated care plans across physical and behavioral health, I/DD, LTSS, and pharmacy, while ensuring trauma-informed and child welfare-aligned care.

Qualifications

• BA/BS degree required with 1 year direct experience with CFSP population or related community-based organizations.

Responsibilities

• Provide integrated whole-person Care Management under the CFSP Care Management model across physical health, behavioral health, I/DD, LTSS, pharmacy, and unmet needs.
• Offer Trauma-Informed Care by recognizing ACEs and coordinating cross-agency care for diverse CFSP needs.
• Collaborate with County Child Welfare Worker to align health care with permanency goals.
• Work with a multidisciplinary care team to coordinate care with primary, behavioral, specialty providers, and child welfare stakeholders.
• Conduct telephonic or in-person assessments to identify and manage member program needs.
• Identify members with potential clinical health care needs and coordinate with clinical teams for care coordination support.
• Oversee non-clinical needs of members with chronic illnesses or disabilities for cost-effective service utilization.
• Set short- and long-term goals with members, caregivers, families, and physicians.
• Identify members who would benefit from expanded services.
