Elevance Health, Inc. is seeking a CFSP Managed Care Coordinator to work in North Carolina field counties, coordinating care for Medicaid CFSP members in Watauga, Ashe, Avery, Wilkes and Caldwell.

This role focuses on seamless, integrated care across physical, behavioral health, I/DD, LTSS and pharmacy with four to five days in person. You will collaborate with County Child Welfare Workers and a multidisciplinary team to assess needs, set goals, and coordinate care, while traveling to worksites

Formación

• BA/BS degree required.
• Minimum 1 year experience with the program population or similar community-based work.
• NC residence preferred; understanding of county child welfare context is a plus.

Responsabilidades

• Provide integrated whole-person Care Management under CFSP care model across health domains.
• Coordinate across physical health, behavioral health, I/DD and LTSS.
• Collaborate with County Child Welfare Workers to align care with permanency goals.
• Engage multidisciplinary care teams to coordinate care for members.
• Conduct assessments by phone or in person to identify member needs.
• Identify members needing expanded services and coordinate with teams.
• Oversee non-clinical needs for members with chronic conditions.
• Set short- and long-term goals with members, families and physicians.
• Ensure cost-efficient and appropriate service utilization.
