Qualifications

• 2+ years in behavioral health, social services, or human services.
• 2+ years with MS Office Suite and multi-system navigation.
• 2+ years in children's mental health, child welfare, or related field.
• 2+ years experience with Ohio delivery systems and local networks.
• Must reside in Ohio and be willing to travel up to 50%.

Responsibilities

• Coordinate and monitor care plans for members in the OhioRISE program.
• Collaborate with Child and Family Teams and providers to address barriers.
• Perform evaluation of needs and eligibility, and adjust care plans accordingly.
• Serve as single point of contact to remediate gaps in care and access.
