CVS Health is seeking a Case Management Coordinator for a full-time, field-based telework role located in Ohio. The position requires travel within Northeast/Northwest/Southeast Ohio to member homes up to 50% of the time, with residency in listed counties including Marion, Union, Portage, Medina, and others.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children’s mental health or related public sector field.
• 2+ years in family systems, case management, or related areas.
• 2+ years with Ohio delivery systems and local networks.

Responsibilities

• Evaluate referred members’ needs/eligibility and recommend an approach to case resolution.
• Coordinate and monitor assigned care plan activities.
• Collaborate with clinicians and care teams to improve care quality and outcomes.
• Identify and escalate quality of care issues and promote member engagement.
• Utilize case management processes to document care and track progress.
