CVS Health is hiring a Field-Based Care Coordinator in Ohio to support the OhioRISE program. This full-time role requires travel within Northeast Ohio to member locations and collaboration with MCOs, vendors, and ODM to deliver coordinated behavioral health services.

The ideal candidate has a bachelor’s or non-licensed master in behavioral health or related field, plus 2+ years of experience in child and family services.

Qualifikationen

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years in behavioral health, social services, or human services.
• 2+ years of experience with personal computers and MS Office applications.
• 2+ years experience in children’s mental health, child welfare, or related fields.
• 2+ years experience with Ohio delivery systems and local networks.
• Willing to travel up to 50% and may travel to New Albany office for trainings.

Aufgaben

• Coordinate and manage care plans for members and ensure timely care.
• Evaluate member needs/eligibility and reference appropriate plans and services.
• Collaborate with care teams and medical directors to optimize outcomes.
• Document care activities and monitor progress within required systems.
