Job summary

CVS Health is seeking a Field-based Care Coordinator in Ohio to support the OhioRISE program, travel to member homes as needed, and coordinate comprehensive behavioral health services.

You will evaluate needs, coordinate care plans, and work with multidisciplinary teams to improve outcomes for children and families. A bachelor's or master's in behavioral health is required, with strong communication and travel willingness.

Qualifications

• Bachelor's degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years in children's mental health, child welfare, or related field.
• Willing and able to travel within Northeast Ohio up to 50%.

Responsibilities

• Coordinate and monitor care plans and progress for members.
• Evaluate member needs/eligibility and identify appropriate services.
• Collaborate with care teams and families to achieve optimal outcomes.
• Provide coaching and support to empower informed health decisions.
