CVS Health is hiring a Case Management Coordinator for a full-time, field-based telework role in Ohio. The position requires travel within the Northeast Ohio region up to 50% of the time and may involve occasional trainings at the New Albany office.

Candidates should reside in the specified counties and have solid experience in behavioral health and case management. The role focuses on care coordination, education, and ensuring healthcare outcomes for members through multi-system collaboration.

Qualifications

• Bachelor's degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers, keyboard and MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or a public sector human services or behavioral health care field.
• 2+ year of experience with Ohio delivery systems, including local community networks and resources.

Responsabilités

• Be clinically and culturally competent/responsive with training and experience necessary to manage complex cases in the community across child-serving systems.
• Evaluate referred members’ needs/eligibility and recommend a care plan.
• Coordinate and implement assigned care plan activities and monitor progress.
• Utilize negotiation and motivational interviewing skills to secure options and engage members.
