Job summary

CVS Health is seeking a Case Management Coordinator in Ohio to coordinate and implement care plans for members across child-serving systems. You will evaluate member needs, collaborate with case managers and medical directors, and engage families to improve health outcomes within a managed care framework.

This full‑time telework position requires travel within Northeast Ohio, with some visits to the New Albany office for trainings.

Qualifications

• Bachelor's degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers, keyboard and MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, or related public sector field.
• 2+ years experience in family systems, case management, or related areas.
• Willing and able to travel within the assigned region up to 50% of the time; mileage reimbursed.

Responsibilities

• Coordinate and implement assigned care plan activities and monitor progress.
• Evaluate member needs/eligibility and determine care resolution approaches.
• Provide coaching, information, and support to empower members to make informed health choices.
• Serve as a single point of contact for members and assist in addressing gaps in care.
