Job summary

CVS Health is recruiting for a full-time Case Manager in Ohio. This role travels within Northeast Ohio to member homes and other locations, supporting care coordination and evaluating needs to inform care plans. Candidates should reside in or near listed counties.

The position requires 2+ years of behavioral health experience, strong MS Office skills, and flexibility to work beyond standard hours while traveling up to 50% of the time.

Qualifications

• Bachelor’s degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years experience with Ohio delivery systems and multi-system navigation.

Responsibilities

• Be clinically and culturally competent with training to manage complex cases in the community.
• Evaluation of members’ needs and coordination of care plans.
• Enhancement of medical appropriateness and quality of care through collaboration with care teams.
• Monitoring, evaluation and documentation of care per regulatory guidelines.
