Elevance Health is seeking a field-based Care Manager in Guilford County, NC. The role involves coordinating integrated care across physical health, behavioral health, and related services, with in-person engagement at client sites four to five days per week.

Responsibilities include assessments, cross‑agency coordination, and identifying members needing expanded services. Prior experience in community-based care and a BA/BS degree are preferred.

Qualifications

• Requires BA/BS degree and at least 1 year of work with the program population.
• Experience with child welfare or related community-based organizations preferred.

Responsabilités

• Provide integrated whole-person Care Management under the CFSP Care Management model.
• Offer Trauma‑Informed Care by recognizing the role of ACEs in the CFSP population and coordinating cross‑agency care.
• Collaborate closely with each Member's County Child Welfare Worker to align health care needs with permanency planning goals.
• Work with a multidisciplinary care team, including primary health care and behavioral health professionals, to coordinate care.
• Conduct telephonic or face‑to‑face assessments using predefined questions to identify, evaluate, coordinate, and manage member program needs.
• Identify members who would benefit from expanded services.
