Qualifications

• At least 2 years health care experience, preferably in behavioral health.
• Licensed behavioral health clinician with active license in state of practice.
• Valid driver's license, reliable transportation for local travel.
• Experience with whole person care, chronic conditions, and discharge planning coordination.
• Data entry skills and proficiency with clinical software and Microsoft Office.

Responsibilities

• Complete comprehensive behavioral health assessments per regulated timelines and determine eligibility for care coordination/case management.
• Develop and implement care plans with member, caregiver, physician and other healthcare professionals.
• Conduct telephonic, face-to-face or home visits as required.
• Monitor care plans to evaluate effectiveness, document interventions and goals.
• Maintain ongoing member caseload for regular outreach and management.
• Promote integration of services across behavioral health, LTSS, and home/community resources.
• Facilitate interdisciplinary care team meetings and ICT collaboration.
• Educate and motivate change using motivational interviewing and Molina clinical guideposts.
• Assess barriers to care and provide care coordination assistance.
• May provide consultation, resources and recommendations to peers.
• 25-40% local travel may be required.
