Job summary

The Jewish Board of Family and Children’s Services is seeking a Care Coordinator to connect adults and children with chronic behavioral health and medical conditions to essential services.

Working in a field-based, team-oriented environment, you will assess risk, develop person-centered care plans, provide care management, track appointments, educate members, and coordinate multiple services to improve health outcomes.

Qualifications

• A bachelor's degree with a major or concentration (minimum of 24 credits) in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing.
• OR NYS teacher's certification for which a bachelor's degree is required; OR NYS licensure and registration as a Registered Nurse and a bachelor's degree plus two years of direct services or case management experience.
• A Bachelor’s Degree, Associates Degree or High School Diploma/GED in another discipline PLUS five years’ experience working with an applicable population.
• Specific experience with the target population may be required to work with Children, Health Home Plus or Adult Health Plus members.

Responsibilities

• Integrate medical, behavioral health, and social support services.
• Periodically assess medical and behavioral health needs and treatment adherence.
• Develop an ICP with the member/family and other providers.
• Provide required care management services.
• Track referrals using HIT systems and ensure access to services.
