Job summary

The Jewish Board in New York City is seeking Care Coordinators to connect adults and children with chronic behavioral health and medical conditions to needed services. You will work in the field and office, assess risks, and develop person-centered care plans to optimize health outcomes.

As part of a growing program, you will coordinate care, educate members, track referrals, and collaborate with families, caregivers, and service providers.

Qualifications

• Bachelor's degree in social work, psychology, nursing, or related field required.
• Two years of direct services or case management for chronically ill or disabled.
• Fluency in a second language preferred (e.g., Spanish, Russian, Creole).

Responsibilities

• Integrate medical, behavioral health, and social services for members.
• Assess medical/behavioral health needs and treatment adherence.
• Develop individual care plans with members, families, and providers.
• Provide required care management services.
• Track referrals using HIT systems.
• Ensure access to ICP-defined services including housing, entitlements, and education.
• Outreach to improve service engagement.
• Respond to member inquiries and referrals.
• Reassess ongoing need for care coordination services.
• Complete documentation and share knowledge with the team.
