Job summary

The Judge Group seeks a Managed Care Coordinator to perform non-clinical management of members' service plans across field settings. This role involves telephonic and in-person assessments, identifying needs, and coordinating with families and interdisciplinary teams to deliver cost-effective care.

Requirements include a BA/BS in a healthcare-related field, at least 1 year of relevant experience, and CHW certification within one year where applicable. Travel to member sites is expected.

Qualifications

• Bachelor’s degree in a healthcare-related field required.
• Minimum of 1 year of experience with the target population or related community-based organizations.
• Community Health Worker (CHW) certification must be obtained within one year for MMP programs.
• Licensure is not required in markets where state law/contracts do not require it.

Responsibilities

• Conduct telephonic or face-to-face assessments to identify members with potential needs.
• Coordinate non-clinical service plans and goals with members, families, and care teams.
• Identify community resources to optimize cost-effective utilization.
• Collaborate with clinical teams to support holistic care delivery.
• Travel to member homes, worksites, and community locations for in-person assessments.
