Job summary

CVS Health is seeking a Call/Case Management Coordinator in Ohio to coordinate care and educate members and families. This field-based role requires deep collaboration with care teams and ongoing support to improve health outcomes.

You will travel within Western Ohio, up to 50%, and may work beyond standard hours to meet member needs. The position requires 2+ years in behavioral health or related fields and a Bachelor’s or Master’s degree in a behavioral health, health services, or public health

Qualifications

• 2+ years in behavioral health or human services.
• 2+ years with computers and MS Office apps.
• 2+ years in children's mental health or public-sector field.
• 2+ years in family systems or case management.
• 2+ years with Ohio delivery systems.
• Must reside in the West Region of Ohio (listed counties).
• Willing to travel up to 50% and occasional trainings at New Albany Office.
• Reliable transportation; mileage reimbursed.
• Willing to work beyond core hours as needed.

Responsibilities

• Coordinate and implement care plan activities; monitor progress.
• Evaluate member needs and eligibility; suggest care approaches.
• Collaborate with care teams and providers to optimize outcomes.
• Provide coaching and support to empower members in decisions.
• Identify and escalate quality of care issues; ensure documentation.
