Job summary

CVS Health Corporation in Ohio is seeking a Field-Based Care Coordination Specialist. This full-time role travels to member homes and other locations across Central and Southeast Ohio, up to 50% of the time, to support managed care services.

Qualifications

• Bachelor's degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years experience in children's mental health, child welfare, or related public sector fields.
• 2+ years experience with Ohio delivery systems and regional resources.

Responsibilities

• Provide care coordination and education for members to facilitate optimal healthcare outcomes.
• Collaborate with case managers, supervisors, Medical Directors and other programs to overcome barriers.
• Monitor and document care plans, eligibility, and services in accordance with policies and regulations.
