Job summary

Molina Healthcare is seeking a dedicated Care Manager to support LTSS services and coordinate integrated member care across settings. This role involves collaboration with multidisciplinary teams, developing and monitoring care plans, and ensuring alignment with regulatory timelines and member outcomes.

Travel of 25-40% may be required; requires licensure as LCSW/LMSW/APSW or equivalent, and strong knowledge of community resources and benefits.

Qualifications

• Minimum 2 years in health care; at least 1 year in care management or LTSS.
• Licensed/credentialed social worker as listed in qualifications.
• Valid driver license and reliable transportation for travel requirements.

Responsibilities

• Complete comprehensive member assessments within regulated timelines and in-person home visits when required.
• Facilitate waiver enrollment/disenrollment processes and develop care plans with stakeholders.
• Monitor and adjust care plans; evaluate outcomes and motivate member progress.
• Promote integration of behavioral health and LTSS; connect to home/community resources.
• Assess medical necessity, authorize services, and discuss funding sources.
