Qualifications

• Licensed clinical social worker or equivalent with active license in state of practice.
• 2+ years in health care; 1+ year in care management or LTSS.
• Experience with disability, chronic conditions, or substance use disorders preferred.
• Bachelor's degree may be required in some states depending on contract.

Responsibilities

• Complete comprehensive member assessments within required timelines, including home visits as needed.
• Facilitate waiver enrollment/disenrollment and develop care plans with members and care network.
• Monitor care plans; document interventions and adjust as needed to meet goals.
• Coordinate with interdisciplinary team to ensure integrated, quality member care across LTSS.
• Advise on funding sources and benefits; assess medical necessity for waiver services.
• Travel 25-40% locally as required by state/contract.
