Job summary

Conduent is seeking a Medical Benefit Review Services Associate to support Louisiana Medicaid in-home assessments and care coordination for older adults and individuals with disabilities. The role requires regular travel within specified parishes and offers field-based work with potential remote elements.

The position emphasizes person-centered care planning, evaluating needs, and guiding clients to state services and community resources within a supportive team environment.

Qualifications

• Bachelor's degree in a related field.
• Strong verbal and written communication skills.
• Interest in working with diverse populations.
• Louisiana resident.
• Reliable transportation.
• Valid driver’s license, current auto insurance, and ability to travel daily.
• Ability to work independently while managing multiple client cases and priorities.

Responsibilities

• Conduct in-home assessments for LT-PCS applicants and participants.
• Evaluate client needs and gather information for eligibility determinations, care planning, and service coordination.
• Provide Choice Counseling for HCBS programs when requested.
• Educate applicants and families about Louisiana Medicaid long-term care programs and community resources.
• Develop and maintain person-centered Plans of Care based on needs and goals.
• Coordinate services and connect clients with healthcare providers and social services.
• Complete monitoring visits, client surveys, case documentation, and quality management activities.
• Maintain accurate records and adhere to state guidelines.
