CVS Health is seeking a behavioral health clinician to apply advanced clinical judgement to assess, plan, and coordinate care for members in Kentucky. You will collaborate with medical and community providers to implement cost-effective, person-centered strategies and support transitions of care.

You will conduct psychosocial assessments, develop individualized care plans, monitor progress, and advocate for resources.

Qualifikationen

• Active, unrestricted licensure as an LCSW, LPC, LMFT, Psychologist, or other qualified behavioral health professional.
• Master's degree in Social Work, Counseling, Psychology, Human Services, or related field.
• Minimum of three years of experience in behavioral health, case management, or care coordination.
• Experience conducting behavioral health assessments and developing member-centered care plans.
• Knowledge of mental health, substance use disorders, trauma-informed care, and community-based services.

• Conduct behavioral health and psychosocial assessments.
• Develop, implement, and monitor individualized care plans.
• Coordinate services across behavioral health, medical, and community providers.
• Support discharge planning and transitions of care.
• Advocate for members and connect them to community resources.
• Monitor member progress and identify barriers to improve outcomes.
• Participate in interdisciplinary care team meetings and provider collaboration.
