Qualificações

• Minimum 2 years health care experience, preferably in behavioral health.
• Licensed behavioral health clinician (LCSW/LMSW/APSW/CHES/LPC/LPCC/LMFT/PhD or PsyD) active and unrestricted.
• Valid driver's license, reliable transportation, and adequate auto insurance for travel.
• Experience with severe and persistent mental health concerns, substance use disorders, and foster care.
• Knowledge of whole person care principles and discharge planning coordination.
• Data entry skills and experience using a clinical platform.
• Excellent verbal and written communication skills.
• Proficiency in Microsoft Office and related software.

Responsabilidades

• Completes comprehensive behavioral health assessments per timelines and determines care coordination eligibility.
• Develops and implements care plans with member, caregiver, and healthcare professionals.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plans, documents interventions, and suggests changes as needed.
• Maintains ongoing member caseload and regular outreach.
• Promotes integration of behavioral health, LTSS, and home/community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Provides education and motivation using clinical guidelines.
• Assesses barriers to care and assists members to address concerns.
• May provide consultation to peers as needed.
• Travel 25-40% may be required.
