Qualifications

• Minimum 2 years in clinical healthcare patient coordination or navigation.
• Strong communication and organizational abilities.
• Familiarity with insurance processes and patient advocacy.

Responsibilities

• Serve as primary contact guiding patients through treatment plans.
• Schedule appointments, tests, referrals; ensure continuum of care.
• Explain medical care, insurance coverage, and financial resources.
• Coordinate communication with providers and administrative teams.
• Maintain accurate EMR documentation.
• Provide emotional support to patients and families.
• Participate in team meetings and quality initiatives.
• Support after-hours call coverage on rotating schedule.
