Job summary

US Fertility is seeking a Fertility Nurse Case Manager to join Fertility Centers of Illinois in Mokena, IL. The role offers a Monday–Friday schedule, 8:00 AM–4:30 PM CT, with occasional weekends and holidays, and a salary range of $72,000 to $84,000 per year, commensurate with experience.

You will coordinate patient care, monitor treatment plans, and ensure accurate communication of orders and lab results. Join a nationally respected organization focused on patient care and outcomes.

Qualifications

• RN required; BSN preferred.
• Current state RN license without limitations required.
• IVF/Fertility experience and background in women’s health preferred.
• Ability to manage a caseload of patients.
• Computer proficiency required.
• Excellent interpersonal skills and ability to work as part of a multi-disciplinary team and build and maintain effective working relationships.
• Strong customer service and results oriented; highly responsive to requests.

Responsibilities

• Assist with coordination, case management, and treatment of fertility patients.
• Monitor patients throughout treatment plans, providing direction and education.
• Communicate physician orders and instructions, as well as laboratory results with interpretation and explanation.
• Establish a compassionate environment by providing support to patients and their families.
• Document patient care services by charting in patient and department records.
