Job summary

Pixel Care in Cedar Knolls, NJ is seeking a compassionate Patient Care Coordinator to support patients through the medication fulfillment process. This full-time, on-site role requires coordinating onboarding, scheduling, refills, and shipments while working closely with Data, Insurance, Pharmacy, Logistics, and other teams.

You will communicate with patients, gather accurate information, document interactions, and help resolve issues with a patient-centered approach.

Qualifications

• High school diploma or equivalent.
• Active NJ pharmacy technician registration.
• Willingness to obtain registrations in other states as needed.
• Strong written and verbal communication with a patient-centered approach.
• Excellent attention to detail and ability to manage a high volume of patient information.

Responsibilities

• Provide compassionate, accurate, and timely support to patients via calls and messages.
• Gather, confirm, and document patient, prescription, allergy, insurance, and other information.
• Coordinate medication scheduling, refills, and shipments.
• Answer patient questions about status, scheduling, and services.
• Educate patients on the fulfillment process and expectations.
• Document patient interactions and scheduling activities accurately.
• Escalate clinical questions to a Pharmacist when needed.
• Collaborate with Data, Insurance, Pharmacy, and Logistics teams.
