Job summary

Talbert House in Cincinnati, OH is seeking a full-time Case Manager for the Fatherhood Program. The role focuses on coaching fathers, coordinating services, and guiding goal setting within the Fatherhood Project.

You will manage caseloads, provide referrals and coordinate resources, supporting clients toward wellness and autonomy while following program protocols and policy standards. First shift with some evening hours.

Qualifications

• Associate degree or equivalent professional experience in behavioral science or related field.
• Ability to learn case management principles and reimbursement requirements.
• Strong organizational, time management and communication skills.

Responsibilities

• Coordinate and provide care safely, timely, and client-centered.
• Maintain caseload with regular contact to monitor progress.
• Draft service plans, review progress, and determine case closures with supervisor.
• Deliver ongoing services including crisis intervention, advocacy and referrals.
• Educate clients and facilitate group curriculum per program requirements.
• Complete documentation to meet program and funder criteria.
• Participate in treatment teams and support client status changes.
