Job summary

Talbert House in Cincinnati, OH is seeking a Case Manager for the Fatherhood Program (Preble County). You will provide individual coaching and guide fathers with goal setting, parenting support, and referrals, while coordinating case management services.

Responsibilities

  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
  • Maintain caseload and monitor progress with regular client contact.
  • Draft service plans and determine case closure with supervisor.
  • Deliver ongoing services including crisis intervention, advocacy, housing assistance, referrals.
  • Assist in wellness through supportive counseling and care coordination.
  • Develop individualized treatment or service plans and ensure policy compliance.
  • Document all clinical activities meeting criteria on time and accurately.
  • Participate in the treatment team and advise on client status changes.
  • Provide client education and facilitate group curriculum.
  • Adhere to professional standards as outlined by protocols and regulations.
  • Other duties as assigned.

Position Summary:

The Fatherhood Case Manager provides individual coaching and is available for fathers who need personalized support. Through the Fatherhood Project, dedicated coaches provide guidance with goal setting, parenting support, resource coordination, referrals, and case management. They trained and facilitate the Nurturing Fathers Program

Position Description:

  • Under minimal supervision, coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered
  • Maintain caseload and utilization by maintaining regular contact with clients to monitor and assess progress
  • Handle case assignments, draft service plans, review case progress and determine case closure for review and/or in consult with supervisor
  • Deliver on-going services to clients which may include crisis intervention, advocacy, medication monitoring, transportation assistance, financial budgeting, housing assistance, and referral and linkage to community resources as needed
  • Assist in helping clients achieve wellness and autonomy through supportive counseling and facilitating multiple care aspects (care coordination, information sharing, etc.)
  • Develop and/or implement individualized client treatment or service plans and ensures compliance with applicable standards and agency policies
  • Document all clinical activities to meet specific program, funder and credentialing entity criteria timely and with accuracy
  • Participate in treatment team and assists in making recommendations for client status changes
  • Provide client education and facilitate group curriculum in accordance with program requirements.
  • Adhere to professional standards as outlined by protocols, rules and regulations
  • Other duties as assigned

For positions assigned to school-based or community programs, up to approximately 30% of annual work time may involve the provision of prevention services, including:

  • Education – Provide group and individual educational services on Mental, Emotional, and Behavioral (MEB) health, Social-Emotional Learning (SEL), coping skills, and related topics to students, families, educators, and community members.
  • Information Dissemination – Share information and resources regarding MEB health issues through school, family, and community events.
  • Environmental– Participate in Positive Behavioral Interventions and Supports (PBIS) efforts and collaborate with community coalitions to support prevention-focused policies and practices.
  • Community-Based Process – Collaborate with schools, community partners, and other agencies to coordinate and support prevention initiatives and services.
  • Problem Identification and Referral– Screen for student, family, and community needs and connect individuals with appropriate services and resources.
  • Alternatives – Encourage participation in positive activities that strengthen protective factors, reduce risk-taking behaviors, and promote healthy attachment to families, schools, peers, and communities.
  • Complete agency, accrediting body, and funder required training
  • Other duties as assigned

Required Knowledge, Skills and Abilities:

  • Ability to learn case management principles, healthcare management and reimbursement requirements

Job Requirements:

  • Associate’s degree in behavioral science or related field, or equivalent work experience as allowed/required by regulatory bodies
  • Driver License and valid auto insurance

Essential Functions/Physical Demands:

  • Positional: Driving in accordance with job duties assigned. Occasional standing and walking. Occasional to frequent sitting.
  • Gross Mobility: Rare climbing, or crawling. Infrequent balancing, stooping, kneeling, or crouching. Frequent to constant reaching andhandling.
  • Sensory: Rare tasting/smelling. Infrequent use of color vision. Occasional use of far visual acuity, depth perception, and field of vision. Frequent use of near and midrange visual acuity, and visual accommodation. Frequent to constant talking.
