Job summary

Catholic Charities in New Haven, CT seeks a Fatherhood Case Manager to provide comprehensive case management, participant support, and administrative coordination for the Fatherhood Program. You will work with fathers to strengthen family relationships, promote stability, and connect them with employment, education, health, and social services.

Responsibilities include outreach, recruitment, facilitation of the 24/7 Dad groups, documentation, data entry, and progress reporting.

Qualifications

Associate degree preferred; equivalent combination of education and relevant experience may be considered.
Experience in social services and case management preferred.
Experience working with fathers, families, underserved populations is highly desirable.

Responsibilities

Establish supportive relationships and assess participant strengths, needs, goals, and barriers.
Facilitate fatherhood education groups and incorporate financial literacy concepts.
Recruit participants, coordinate outreach, marketing, and community engagement.
Complete required documentation, including assessments, service plans, case notes, referrals, and reports.
Monitor progress, ensure enrollment goals and program outcomes are achieved.
Identify patterns and trends within the program and report to supervisor.
