Job summary

Fathers & Families Support Center in Missouri seeks a Full-Time Case Manager to assess fathers, develop self-sufficiency plans and connect them with community resources. The role emphasizes advocacy, counseling, and program delivery across multiple sites.

Responsibilities include conducting social assessments, facilitating groups, and maintaining records. Office travel is required, with occasional evenings and weekend events as noted in the description.

Qualifications

• Must have effective verbal and written communications and interpersonal skills.
• Familiar with MS Office Products.
• Must be able to organize and present information and work independently.
• Must have strong case management and documentation skills.
• Must understand and practice the NASW code of ethics.

Responsibilities

• Complete a social assessment and develop an individualized plan for self-sufficiency and responsible fatherhood.
• Assist client in overcoming barriers through intervention, advocacy, and referrals.
• Identify achievable goals and provide personal support to participants.
• Facilitate peer and family support groups and coordinate topics and speakers.
• Travel to multiple locations and conduct program activities.
• Maintain ongoing case management and weekly contact with graduates for one year.
• Document records and forms to ensure accurate tracking.
• Develop and maintain a network of community resources and social supports.
• Attend staff and department meetings and client graduations.
