Job summary

Talbert House is seeking a Case Manager for the Fatherhood Project in Cincinnati to empower fathers, strengthen families, and support personal growth. You will provide coaching, planning, and connections to community resources, helping clients set goals and move toward independence.

Under supervision, you will manage an active caseload, develop service plans, and collaborate with a care team to ensure quality, compliant services.

Qualifications

• Associate degree in behavioral science or related field.
• Valid driver's license and current automobile insurance.
• Ability to meet applicable agency and program requirements.

Responsibilities

• Build meaningful relationships with fathers and families.
• Manage an active caseload and monitor progress.
• Develop individualized service plans with clients.
• Provide hands-on support for navigating life challenges and resources.
• Coordinate services with community agencies and providers.
• Maintain timely, accurate documentation of services.
• Collaborate with care team and participate in program meetings.
