Job summary

UPMC Community is seeking a Medication Transition Coordinator I to support medication transitions for patients during hospital admissions and discharges, and select outpatient visits, with the goal of ensuring patients have medications and requested products prior to leaving the hospital.

This role will operate in the emergency department and requires independent work, flexibility, and clear communication with patients, families, and the care team; schedule includes two 12-hour shifts and two

Qualifications

• Obtain and document complete medication histories.
• Introduce the Meds-to-Bed program to patient.
• Deliver prescription medications and products to bedside.
• Coordinate with case managers on insurance and payment.
• Maintain HIPAA Privacy Standards and Requirements.
• Work autonomously in a fast-paced hospital setting.

Responsibilities

• Obtain and document complete and accurate medication histories.
• Introduce the Meds-to-Bed program to patient.
• If patient agrees, verify eligibility and payment method for services.
• Deliver medications/products to patient bedside once fulfilled by Pharmacy.
• Collaborate with clinical team on post-discharge medications and self-care products.
• Assist patients with financial concerns by coordinating with case management.
• Ensure insurance information, prescriptions and orders are sent to the pharmacy.
• Demonstrate professionalism, organization, and sound judgment in a fast-paced environment.
