Job summary

Qualifications

• Pharmacy Technician Level 1 qualification or equivalent.
• Ability to work independently and contribute to effective working relationships.
• Experience managing projects is preferred.
• Competency with Microsoft Office Suite and patient data handling.

Responsibilities

• Conduct comprehensive medication history interviews with patients, caregivers, and external sources.
• Document home medications with drug name, dose, route, frequency, and last dose taken.
• Identify discrepancies and potential issues for pharmacist review.
• Prioritize high‑risk patients for timely reconciliation in the ED.
• Communicate effectively with acutely ill patients and those with limited health literacy.
• Explain the medication history process and its importance to patient safety.
• Escalate complex issues to the pharmacist as needed.
• Ensure HIPAA compliance and adherence to institutional guidelines.
• Maintain thorough, timely documentation to support workflows.
