Job summary

UPMC East is hiring a part-time Medical History Specialist to join the team. The position is scheduled for 24 hours per week (three 8-hour shifts, 10 am to 6 pm) with rotating Saturday responsibilities.

Under supervision of the Emergency Department Director and Clinical Pharmacy staff, you will compile medication histories for admitted patients and ensure accurate entry into the EHR for reconciliation purposes.

Qualifications

• High School Diploma or equivalent.
• Two or more years of experience in related role preferred.
• Certified Pharmacy Technician (CPhT) preferred.
• PA State Board of Pharmacy Registration required for all PA locations.
• Act 31, Act 33, Act 34, Act 73 clearances required/renewals.

Responsibilities

• Constructively receives feedback and direction; identifies learning needs and plans improvements.
• Answers inquiries with accurate information or directs to appropriate resource.
• Communicates delays in reconciliation process and troubleshoots for future improvements.
• Directs unresolved issues to the admitting provider.
• Communicates with other health care providers to prioritize patient care.
• Updates medication list to support admission orders.
• Gathers information from patient interviews, physicians, community pharmacies and facilities for accurate entry.
