UPMC Community is seeking a Medication Transition Coordinator I to support patient medication transitions during hospital admissions and discharges, and select outpatient visits. The role operates in the emergency room and May require independent work.

Schedule includes two 12-hour shifts and two 8-hour shifts weekly with rotating weekends and holidays. The coordinator will collaborate with clinicians and case managers to ensure medications and related products are ready at discharge.

• Demonstrates strong communication with patients and staff.
• Maintains HIPAA privacy standards and professional boundaries.
• Able to work autonomously in a fast-paced hospital setting.

Responsabilidades

• Obtain and document complete and accurate medication histories.
• Introduce the Meds-to-Bed program to patients.
• Ensure patient consent, eligibility, and payment method for services.
• If enrolled, update pharmacy of choice in the electronic system.
• Collaborate with the clinical team for post-discharge medications and self-care products.
• Assist patients with financial concerns by coordinating with case management.
• Send insurance information, prescriptions and orders to the pharmacy.
• Deliver prescribed medications and products to the patient’s bedside after fulfillment.
• Collect payment and encourage counseling sessions with the Pharmacist.
• Adhere to company policies and HIPAA Privacy Standards.
• Maintain knowledge of equipment, services, and supplies for transitioning patients.
• Adapt to flexible locations and schedules in a fast-paced hospital environment.
