Job summary

Racker's Community Services team is seeking a Family Support Coordinator Team Leader to oversee and coordinate community-based support services for individuals with developmental disabilities and their families. In this leadership role you will supervise the Family Support Coordinator team, mentor staff, and ensure high-quality, person-centered services that promote independence and community inclusion.

Qualifications

Bachelor’s degree preferred, or a combination of education and experience.
Ability to meet driver criteria or hold a valid driver's license.
Excellent interpersonal and organizational skills.
Strong communication and multitasking abilities in a fast-paced environment.

Responsibilities

Directly responsible for oversight and supervision of the Family Support Coordinator team.
Mentor and coach peers; serve as designee when supervisor is unavailable.
Coordinate community-based support projects and programs.
Build strong relationships with families and community partners.
Ensure timely, quality services and documentation.
Attend Life Plan and COS meetings as needed.
Develop staff action plans and monitor progress.
Maintain confidential staff and participant files.
