Job summary

Health Connect America is seeking an Independent Contractor for Family Support Services. The role focuses on improving family functioning, coordinating referrals, and delivering case management activities including supervised visitation and parenting support.

The contractor will work with DFCS-involved families, complete in-home visits, and collaborate with schools and therapists to ensure service alignment. A background check is required, and payments are invoiced hourly or per task.

Qualifications

• Bachelor’s degree in a human services-related field or a HS diploma with 5+ years of relevant experience.
• Strong understanding of family systems and trauma-informed care.
• Knowledge of community resources and referral networks.
• Reliable transportation, valid driver’s license, and proof of insurance.
• Willingness to complete background checks and contractor onboarding.

Responsibilities

• Coordinate with community providers to facilitate comprehensive care for families.
• Participate in collaborative case planning and review meetings.
• Deliver case management activities including supervised visitation and parent aide support.
• Provide transportation related to family service plans and appointments.
• Maintain thorough documentation and billing logs in accordance with agency standards.
