Health Connect America is seeking a qualified Independent Contractor-Family Support Services to enhance family functioning and child well-being through in-home visits and community collaboration. You will coordinate with schools and therapists, provide case management, supervised visitation, and parent education, and document services for billing.

A Bachelor’s in human services or equivalent experience with trauma-informed approach is required, plus reliable transportation and background checks.

Qualifikationen

• Bachelor’s degree in human services or related field.
• Or High School Diploma with 5+ years of relevant experience.
• Trauma-informed care and knowledge of community resources.
• Reliable transportation, valid driver’s license, and proof of insurance.
• Willingness to complete background checks and onboarding.

Aufgaben

• Coordinate with community providers to facilitate care and referrals.
• Participate in collaborative case planning and case review meetings.
• Deliver case management activities including supervised visitation, parent aide, and transportation.
• Conduct in-home visits to monitor progress and support families.
• Maintain documentation and service logs aligned with billing and reporting standards.
• Provide parenting education and modeling positive parenting strategies.
