Health Connect America is seeking an Independent Contractor to provide Family Support Services designed to improve family functioning and child well-being. You will collaborate with families, case managers, and community partners to implement individualized service plans.

The role requires a background in human services and a trauma-informed approach, with strong coordination and documentation responsibilities.

Qualifikationen

• Bachelor’s degree in a human services-related field or 5+ years of verifiable field experience.
• Strong understanding of family systems and trauma-informed care.
• Reliable transportation, valid driver’s license, and proof of insurance.
• Willingness to complete required background checks and contractor onboarding.

Aufgaben

• Coordinate with community providers to facilitate comprehensive care and referrals.
• Participate in collaborative case planning and case review meetings, when requested.
• Deliver case management activities including supervised visitation, parent aide support, and Behavioral Aide.
• Maintain documentation of services and billing logs in accordance with agency expectations.
• Provide parenting education and modeling of positive parenting strategies.
• Coordinate with external professionals to ensure service alignment.
• Complete in-home visits to monitor progress and provide ongoing support for families.
