Job summary

Health Connect America is seeking an Independent Contractor-Family Support Services position to improve family functioning and child well-being. A Bachelor’s degree in a human services-related field, or a HS Diploma with 5 years of field experience, is required to deliver case management, supervised visitation, parent aide support, transportation, and crisis intervention.

Qualifications

• Bachelor’s degree in a human services-related field or HS Diploma/GED with 5 years of verifiable experience in the field.
• Strong understanding of family systems and trauma-informed care.
• Reliable transportation with valid license and insurance.

Responsibilities

• Coordinate with community providers to facilitate comprehensive care and referrals.
• Participate in collaborative case planning and review meetings.
• Deliver case management activities: supervised visitation, parent aide support, behavioral aide, transportation, crisis intervention.
• Advocate and link to services, including drug screening if applicable.
• Complete in-home visits to monitor progress and provide ongoing support for families involved with DFCS.
• Maintain documentation of services and logs in accordance with agency expectations.
